Family Outreach
Family Outreach is a community-based program aimed at helping families access essential resources and support services. Upon receiving a referral, Family Outreach works to link individuals with specific programs and resources in their community that are tailored to their needs. Resources may include housing, financial support, clothing, food security, and behavioral health services.

Family Outreach offers a family-centered approach based on each family’s unique strengths and needs. Once the family assessment is completed, the family will be referred to a community provider who will help to obtain identified resources needed.
Our Process
When a referral indicating that a family needs additional support or assistance, we make sure to connect them with the Family Outreach team.
- A Family Advocate is assigned to the family. The Family Advocate will reach out and initiate contact with the family.
- Together, the Family Advocate and the family will complete a detailed questionnaire to identify specific needs.
- The Family Advocate will use the this information to work with local, community-based, providers and/or agencies.
- Meetings are scheduled to introduce the family to the community provider and kickstart the services and resources, providing a warm hand-off to providers.
- Family Advocates work closely with the family to provide or connect them with essential resources such as housing, budget management, financial resources, clothing, food, and behavioral health services
